MIGS – Minimally Invasive Glaucoma Surgery

Minimally Invasive Glaucoma Surgery (MIGS) is a group of advanced surgical procedures that lower intraocular pressure with less risk and faster recovery than traditional glaucoma surgery. At SightMD, MIGS is part of our comprehensive approach to dropless glaucoma care, offering an excellent option for patients who need long-term pressure control with fewer medications.

What is MIGS?

MIGS procedures involve microscopic devices or techniques that improve fluid drainage from the eye with minimal disruption to tissue. These procedures are often combined with cataract surgery and are ideal for patients with mild to moderate glaucoma.

Types of MIGS Offered at SightMD:

  • iStent Inject®: A tiny device inserted into the drainage canal to improve fluid flow
  • Hydrus Microstent: A flexible scaffold placed in Schlemm’s canal to enhance outflow
  • OMNI Surgical System: A comprehensive procedure that addresses multiple points of resistance in the drainage system
  • Kahook Dual Blade: A bladeless tool that removes obstructive tissue in the trabecular meshwork

Who is MIGS for?

MIGS may be right for:

  • Patients with mild to moderate open-angle glaucoma
  • Individuals undergoing cataract surgery who also need glaucoma treatment
  • Patients seeking alternatives to long-term medication use

Benefits of MIGS

  • Minimally Invasive: Less risk than traditional glaucoma surgery
  • Quick Recovery: Most patients return to normal activities within days
  • Can Be Combined with Cataract Surgery: One procedure, two solutions
  • Long-Term IOP Control: Reduces reliance on glaucoma medications
  • Customizable: Tailored to each patient’s specific anatomy and condition

The Procedure: What to Expect

MIGS procedures are performed at one of SightMD’s ambulatory surgical centers. Depending on the type of MIGS and whether it’s combined with cataract surgery:

  1. You’ll receive local anesthesia.
  2. The surgeon will insert a tiny device or perform a gentle modification to your eye’s drainage system.
  3. The procedure typically takes 15–30 minutes.

Recovery and Follow-Up

Postoperative care includes antibiotic and anti-inflammatory drops. Your doctor will monitor your pressure and healing closely during follow-up visits. Most patients see significant improvement in pressure and a reduction in their medication burden.
